Incident Investigation Checklist and Instructions

Want to do a better job in your incident or accident investigations? Here are two forms in PDF format that will help. One is a checklist that will assist you in investigating an accident or related incident; get the file.  When you need to conduct this type of investigation, we also suggest you download the companion document, How to Investigate an Accident. Forms for Calculating Costs of Street Sweeping

These handouts were originally developed by request of contractors who were interested in helping municipalities get a handle on street sweeping costs so they could bid to privatize sweeping. Over time, the handouts have seen more use by both sweeping contractors and municipal agencies trying to get a handle on their own respective costs.

There are PDF two forms, one for computing cost per curb mile and, secondly, for computing cost per hour.
